If you have not used this system before, EVEN IF YOU HAVE A PREVIOUS OR EXISTING JACK CREEK ROAD PASS, do not use this form. Instead, you must complete our New Contractor Request Form.

• Please allow 24-48 hours for your pass to be received and reviewed. Passes are processed Monday-Friday, 9AM-5PM. Requests received after 3pm on fFriday are considered received on Monday AM.

• Your application is not complete until you have submitted all required paperwork and a completed online liability waiver.

• We will contact you if more information is required to complete your request.

• We reserve the right to refuse access to anyone.

• Once your application is approved, you will receive a link and instructions on how to make your payment. Once payment is received, your pass will be emailed to you.
